This page explains your rights as a client of Quiet Roots Therapy, PLLC and provides important information about accessing your mental health records, contacting the Texas Behavioral Health Executive Council (BHEC), and filing a consumer complaint with the Texas Office of Attorney General. It is required by Texas law (HB4224, effective September 1, 2025) to help you understand how to protect your rights and get information about your care. Please read this page carefully and use the links and instructions below if you need to require records or report a concern.
Request Your Health Records
Clients may request copies of their mental health records at any time by completing the Release of Information (ROI) form in the secure client portal. Once signed and submitted, your request will be processed. If you don't yet have access to the client portal, please contact 682-325-8052 for portal setup.
